Dear Parent or Guardian,

Thank you for your interest in 2022 Sertoma Camp Endeavor. We are open to Deaf and Hard of

Hearing children, children of Deaf adults (CODAs) who are hearing, and siblings of D/HH

children. All children must be between the ages of 7 and 17.

Camp Endeavor will run for one week for this age group, Sunday July 17th - Saturday July 23th, 2022.

Check-in on Sunday July 17th, 2022 will be 2:00-4:00 p.m. Please do not arrive and expect to drop off your

camper(s) any earlier than 2:00PM as we will be properly preparing for camp to open.

We strive to provide a safe and fun atmosphere for our campers (more on Covid-19 safety precautions

below).

Camp will end on Saturday, July 23rd.

CAMPERS MUST BE PICKED UP BY 2:00PM.

This year, there will be a fee of $200.00 for every child. A check or money order for $200.00

camp tuition must be sent in with the enclosed registration form. All checks or money orders are

to be made out to CAMP ENDEAVOR and in care of C.H.I.P.S. (109 NE Crystal St, Suite B Crystal River,

FL 34428).

Your child’s registration form MUST include a copy of their MEDICAL INSURANCE CARD and the

CAMP PHYSICAL FORM (included). The physical form MUST be completed by a licensed physician

that verifies your child’s health.

**PLEASE BRING A COPY OF YOUR INSURANCE CARD TO CAMP WHEN YOU ARRIVE!!

Your child cannot be accepted to camp without insurance coverage.

The registration deadline is June 24th, 2022 (or until camp is full). Physical forms and

payment must be received by July 1st, 2022. If a physical form or payment is not received on

time, your child may lose their spot.

COVID-19/CDC GUIDELINES WILL BE FOLLOWED AND IMPLIMENATED IN CAMP. WE WILL

KEEP YOU UPDATED TO RULES ISSUED AS CAMP TIME ARRIVES.
